Therapy 2000 in Houston, TX seeks a Pediatric Physical Therapist to provide home-based therapy for children, using standardized tests to assess needs and develop individualized treatment plans. You will create home exercise programs with caregivers and coordinate care across the patient’s medical team.

The role requires at least 3 years of pediatric PT experience, home-health exposure, and bilingual English/Spanish capabilities.

Qualifications

• 3+ years as a pediatric Physical Therapist.
• Experience in a home-health setting.
• Specialty Certifications (example: PCS)
• Bilingual (English/Spanish)

Responsabilités

• Perform comprehensive Physical Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physician orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Met deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Met with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric physical therapy.
