Job summary

Alternate Solutions Health Network is seeking a licensed Occupational Therapist to provide home-based therapies across the Akron area. You will work with an interdisciplinary team to deliver quality patient care under physician orders, with a focus on safety, documentation, and family education.

The role requires a current OT license, at least one year in an acute setting, and a valid driver's license. We offer a guaranteed base salary plus an uncapped bonus, mileage reimbursement, flexible

Qualifications

• Active Occupational Therapist license in the state of employment.
• Minimum one year of OT experience in an acute care setting; home care preferred.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Plan of Care: provide skilled OT as ordered by physician and document progress.
• Evaluation: assess patient functioning and prognosis.
• Patient Outcomes: use home exercise programs to improve ROM, strength, and coordination.
• Minimize Patient Risk: reduce re-hospitalizations through education and preventive care.
• Family Educator/Advocate: counsel patient and family on disease and home management.
• Supervision: supervise COTA as needed.
• Communication/Collaboration: coordinate with the care team.
• Policies: document according to agency and Medicare guidelines.
• Rules and Regulations: follow agency policies and inform scheduling of changes.
• Operations: participate in in-service trainings and meetings.
