Job summary

@Home Therapy is seeking a licensed Occupational Therapist (OT) to provide home-based occupational therapy services to adult and geriatric patients throughout North Queens, NY. This role offers flexibility and the opportunity to work one-on-one with patients in their homes.

You will perform in-home evaluations, assess ADLs/IADLs, develop individualized treatment plans, train patients and caregivers in adaptive equipment and safety, and document progress in EMR while coordinating with the care

Qualifications

• Licensed NYS Occupational Therapist.
• Strong communication and clinical reasoning skills.
• Comfortable working independently and documenting in EMR.

Responsibilities

• Perform occupational therapy evaluations in the home.
• Assess ADLs, IADLs, upper extremity function, cognition, coordination, home safety, and functional independence.
• Develop individualized treatment plans to improve daily function and quality of life.
• Train patients and caregivers in adaptive equipment, energy conservation, fall prevention, and safe performance of everyday activities.
• Monitor patient progress and modify treatment plans as needed.
• Complete timely EMR documentation and collaborate with physicians and the interdisciplinary care team.
