Job summary

U.S. Department of Veterans Affairs is seeking an Occupational Therapist for the Home Based Primary Care (HBPC) program in the Northeast Ohio VA Healthcare System.

The role focuses on interdisciplinary care for veterans with complex medical needs in the home setting, conducting functional evaluations and planning OT interventions. The incumbent will work under the HBPC supervision to develop and implement therapy plans, coordinate care with other health professionals, and ensure comprehensive

Qualifications

• NBCOT certification required with active status.
• License to practice OT in a state/territory/DC.
• Proficiency in spoken and written English.
• Valid driver’s license required.
• Ability to work in an interdisciplinary team.

Responsibilities

• Assist HBPC therapy supervisor in planning and developing treatment programs.
• Develop and implement monitoring of quality of care within HBPC.
• Assess patients for dysfunction and create measurable goals.
• Maintain contact with other health care providers involved in patient care.
• Complete patient care documentation in a timely manner in accordance with VA policy.
• Enter consults to Prosthetics and coordinate with HBPC services.
• Provide expertise in planning OT services for patients.
• Maintain licensure and pursue continuing education.
• Educate facility staff on HBPC services and programs.
