The role emphasizes direct patient care, health assessments, and care management within an integrated care model, requiring licensure, DEA, CPR/BLS, and strong EHR skills.

Qualifications

• Current Nurse Practitioner license required.
• Active DEA license or eligibility.
• CPR or BLS certification required.
• Valid driver's license with good driving record.
• Outstanding EHR skills.

Responsibilities

• Lead setup and rollout of new markets and training.
• Support market launches and establish local care teams.
• Develop and implement clinical workflows in new markets.
• Provide on-site training and mentorship to clinicians.
• Deliver direct home-based patient care and care management.
