Collage Home and Community Rehabilitation Partners is seeking experienced Licensed Clinical Social Workers to provide truly patient-centered care in New Braunfels, TX, and surrounding areas, with PRN options available. Our interdisciplinary teams deliver in-home and community-based treatment plans for neurological injuries and complex orthopedic diagnoses, aiming for durable, functional outcomes.

Qualifications

• Master's degree in Social Work required.
• Active LCSW licensure required in Texas.
• Excellent planning, organization, writing, and time management skills; ability to communicate with families, professionals, and co-workers.

Responsabilités

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to assist with adjustment to disability, resource linkage, and behavior modification
