Job summary

Interim HealthCare is seeking a Medical Social Worker (MSW) to visit patients in their homes across Ormond Beach, FL and nearby areas. You will connect patients and families with resources, counseling and support to help them overcome health challenges and improve quality of life.

You will work with a home health team to assess psychosocial needs, provide counseling, plan resources, and advocate for patients and caregivers while maintaining a patient-centered approach and strong communication.

Qualifications

• Master’s degree in Social Work and active MSW license in Florida.
• Minimum of 1 year of social work experience in home health or related field.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Strong interpersonal skills, good communicator, empathetic, compassionate and resourceful.

Responsibilities

• Provide home-based social services to patients in their homes, focusing on plan of care and goals.
• Work with an interdisciplinary home health team (RN, LPN, CNA, HHA, PT, OT, SLP) to support patient outcomes.
• Monitor psychosocial conditions and identify needs.
• Conduct patient assessments, document progress, and update plans of care.
• Provide counseling, community resource planning, crisis intervention and advocacy.
• Consult with family and caregivers on the plan of care and progress.
