Job summary

UnitedHealth Group, part of Elk Valley and LHC Group family, is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes under physician plans and coordinate care within the health team.

The role requires an active LPN license in the practice state, current CPR certification, and reliable transportation. Hourly pay ranges from $20.38 to $36.44, with a comprehensive benefits package and opportunities for growth.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• Valid driver's license and access to a dependable vehicle, or reliable transportation.
• Ability to function with diverse patient populations.

Responsibilities

• Provides high quality clinical services within scope of practice, as defined by state laws governing nursing, in accordance with the plan of care.
• Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
• Actively participates in the agency's care coordination process, communicating effectively with patient, family, physician, other clinicians and case coordinators.
