Elk Valley, part of LHC Group, seeks a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients’ homes and document clinical progress under physician orders. You will coordinate other patient care activities to ensure high-quality care in your service area.

Qualifications

• Must hold a current and unrestricted LPN license in state of practice.
• CPR certification preferred or to be obtained within 90 days of hire.
• Strong communication, writing, and organizational skills.
• Ability to work independently and manage multiple tasks.

Responsibilities

• Provide direct nursing care as prescribed by physician to acutely or chronically ill patients in their homes.
• Record clinical and progress notes; coordinate other patient care activities/services.
• Document patient visits thoroughly in electronic medical records.
• Function effectively in diverse home settings and with varied client needs.
