Job summary

Qualifications

• Master’s Degree in Social Work required.
• Licensed Clinical Social Worker (LCSW) licensure per IDFPR.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
• Develop bereavement plan of care addressing family needs.
• Document psychosocial assessments and MSW interventions in the EMR.
• Educate patients and families to overcome barriers and support self-determined care decisions.
• Coordinate with the IDT and facilitate referrals to community resources.
