Job summary

Qualifications

• Licensed Clinical Social Worker (LCSW) licensed by the Illinois Department of Financial and Professional Regulation (IDFPR).
• Driver's license issued by the Illinois Secretary of State (ILSOS).
• Proof of auto insurance as required.
• Licensed Social Worker (LSW) licensed by the Illinois Department of Financial and Professional Regulation (IDFPR).
• Education: Master’s Degree in Social Work; hospice work experience preferred.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plans of care addressing family member needs.
• Document assessments and social work interventions in the EMR.
• Coordinate with interdisciplinary teams to support patient care goals and end-of-life decisions.
• Assist with transportation barriers and referrals for social services.
