Job summary

VNS Health is seeking a Home Care RN to provide personalized nursing and care management to patients at home or in care facilities across New York. You will work with an interdisciplinary team, assess changing needs, and adjust plans of care to ensure optimal outcomes.

Candidates must hold a NY RN license, have at least one year of medical/surgical nursing, and obtain wound care certification within a year. Hospice/palliative certification is preferred.

Qualifications

• Current license to practice as a Registered Nurse in New York State.
• Minimum of one year nursing experience in a medical/surgical environment.
• Wound Care Certification or willingness to obtain within one year of hire.
• Certification in Hospice and Palliative Care Nursing preferred.

Responsibilities

• Practice independently in the community as part of an interdisciplinary care team.
• Deliver personalized nursing and care management to patients in their home or care facilities.
• Constantly evaluate evolving patient needs and respond with plan of care adjustments.
