Job summary

Constellation Health Services is seeking a Nurse Practitioner to provide palliative and hospice care on a PRN basis in the Waterville, Maine area. The NP will assess, diagnose, plan, and manage hospice care under physician supervision.

Responsibilities include face-to-face visits, eligibility determinations, coordinating with the hospice interdisciplinary team, educating patients and families, and maintaining medical records.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current DEA license and state licensure; CPR certification per state regulations.
• Experience in home care/hospice required.
• Accreditation from an approved certifying body for advanced practice nursing.
• Certification in Hospice and Palliative Nursing (CHPN) preferred.
• Excellent observation, verbal and written communication; problem solving.
• Must be able to lift, position and transfer patients; valid driver.

Responsibilities

• Provides medical care, palliates and manages terminal illness under physician supervision.
• Completes history and physical examinations and documents findings.
• Develops an individualized plan of care; prescribes medications and coordinates care.
• Assesses hospice eligibility and supports recertification with the hospice team.
• Collaborates with hospice interdisciplinary group to plan and deliver care.
• Educates patients and families about care, prevention, and end-of-life issues.
• Maintains accurate medical records and compliance with HIPAA.
• Participates in team meetings and supports program growth.
