Job summary

Hartford HealthCare is seeking a full-time Nurse Practitioner or Physician Assistant to join the REACH program delivering primary care in geriatric patients in their homes and in assisted living facilities around greater Hartford, CT.

You will work with a multidisciplinary team to provide comprehensive care, manage treatment plans, and support families. NP/PA candidates must have CT licensure or eligibility and experience in geriatrics or primary care; two years preferred.

Qualifications

• Unrestricted CT license or eligibility for APRN/PA.
• Experience in geriatric or primary care settings preferred.

Responsibilities

• Perform routine physical examinations in home or facility settings.
• Develop care plans and education for patients and families.
• Collaborate with physicians and multi-disciplinary care team.

We are seeking a full-time Nurse Practitioner or Physician Assistant for our REACH (Resource for Elderly and Care at Home) Program, delivering compassionate primary care in the comfort of geriatric patients’ homes and assisted living facilities. Nurse Practitioners And Physician Assistants Role Scope

• Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.


• In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.


• Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physicians.

Qualifications

• Licensure: Unrestricted State of Connecticut Medical License (or eligible)


• Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA


• Experience: At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA). Primary Care/Geriatric Medicine experience highly preferred.
