George Junior Republic and Affiliates seeks a Clinical Case Manager to provide home- and community-based quality services for families involved with the Department of Child Services and Juvenile Probation in Shelbyville, Indiana. The role requires maintaining confidential records, completing intake forms, and delivering services per Indiana standards.

You will document client contacts, provide monthly progress reports, and coordinate treatment and safety plans while participating in court

Formación

• Excellent written and verbal communications skills are required.
• Strong organizational skills and attention to detail are essential.
• Bachelor’s or Master’s degree from an accredited university is required.

Responsabilidades

• Ensure all case records, correspondence, and conversations remain confidential per HIPAA regulations.
• Complete releases and intake forms upon initial contact and save copies in the clinical record.
• Provide home-based services to clients in accordance with Indiana Service Standards.
• Document client contacts in Case Management Pro, VPN client files, and Kid Traks as applicable.
• Provide monthly Progress Reports by due date to supervisor, referring agencies, and Kid Traks if applicable.
• Develop Treatment Plans, Safety Plans, and other required documents within specified timelines.
• Complete visitation/parenting time logs and submit to referral sources within 48 hours when applicable.
• Oversee assessments such as PFS-2, NCFAS, UCLA PTSD, IARCA packets at key milestones.
• Record billable and non-billable time daily and maintain accurate timesheets.
• Adhere to evidence-based models approved by Indiana and George Junior Republic.
• Submit court appearance requests monthly to Compliance Coordinator.
• Attend court hearings, case conferences, and staffings as needed.
• Use strength-based approaches to build protective factors and parenting skills.
