Job summary

LifeWorks NW seeks a Care Coordinator to provide clinical and administrative support in a medical/psychiatric setting at the Rockwood location. The role focuses on coordinating care, performing wellness assessments, and linking clients to medical and psychiatric services to improve access and outcomes.

The position requires a bachelor’s degree in a behavioral science or equivalent experience, strong computer skills, and cultural competence.

Qualifications

• Bachelor's degree in a behavioral science field from an accredited college/university OR three (3) years of full-time professional experience working with adults experiencing mental illness.
• Demonstrates cultural competency and trauma-informed care in addressing barriers to access and engagement.
• Good computer skills including Microsoft Outlook, Word, OneDrive, Excel and Teams; ability to learn EHR systems.

Responsibilities

• Provide Wellness Assessments to clients to support engagement with providers and whole-person health.
• Coordinate referrals with other providers in a timely and professional manner.
• Collaborate with interdisciplinary teams to address medical, oral health, and psychosocial needs.
• Maintain confidentiality and complete accurate documentation in the EHR system.
• Travel between sites as needed and meet driving requirements.
