Job summary

Catholic Charities in Spokane is hiring for a housing stability specialist to assist individuals in the Supportive Services program. You will help clients obtain and maintain permanent housing, develop plans, coordinate resources, and support program operations.

The role requires engaging clients, completing assessments, and maintaining accurate HMIS records while adhering to funder guidelines and HIPAA policies. This position emphasizes compassionate service and community partnerships.

Qualifications

  • Education/Experience: bachelor’s degree in social services or related field and minimum one year of relevant experience; OR AA with 3 years relevant experience.
  • Experience in low-barrier homeless services or mental health preferred.

Responsibilities

  • Develop client centered housing stability plans for caseload and work closely with those individuals to facilitate movement into permanent, stable housing.
  • Complete relevant assessments according to funder/contractual guidelines.
  • Facilitate connections to community resources by developing partnerships and up-to-date resource knowledge for individuals experiencing homelessness.
  • Connect clients with vocational services, SSI/SSDI and public benefits where applicable.
  • Fill in for respite staff as needed to support client care.
  • Provide FCS case management support including assessments, service coordination, referrals, documentation, and billing.
  • Maintain consistent client engagement by proactively contacting clients in shelters and program spaces.
  • Maintain legible client files; ensure HMIS data is current and real-time.
  • Complete monthly reviews of data relevant to the project and report to supervisor.
  • continually seek improvements in HOC operations and case management program.
  • Maintain professional boundaries and self-care.
  • Maintain training to keep up-to-date practices like progressive engagement and motivational interviewing.

Job Summary

This position is responsible for assisting individuals enrolled in HOC’s Supportive Services program in obtaining and maintaining permanent, stable housing.

Job Duties/Responsibilities

To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. Reasonable accommodation may be provided to enable individuals with disabilities to perform the essential duties/responsibilities of this job.

  1. Develop client centered housing stability plans for caseload and work closely with those individuals to facilitate movement into permanent, stable housing, utilizing housing first principles and progressive engagement.
  1. Complete relevant assessments according to funder/contractual guidelines.
  2. Facilitate connections to appropriate community resources by developing and maintaining good community partnerships and up to date knowledge of resources for individuals experiencing homelessness.
    1. Specifically, provide resources on landlord vacancies and work directly with clients to mitigate evictions, poor credit and criminal histories.
  1. Connect clients with vocational services, SSI/SSDI and public benefits, where applicable.
  1. Fill in for respite staff as needed to support client care and program operations.
  1. Provide FCS case management support, including assessments, service coordination, referrals, documentation, and billing.
  1. Support the RC Program by enrolling clients in the program and assisting them in achieving employment readiness goals through skills assessment, employment-focused mentoring, job search support, resume and interview preparation, and referrals to vocational training programs and employment opportunities.
  1. Maintain consistent client engagement by actively seeking out assigned clients, initiating contact within the shelter and program spaces, and using progressive engagement strategies to meet required client engagement expectations.
  1. Maintain legible and complete client files according to funder requirements. Ensure client data in HMIS is current, accurate and entered in real time.
  1. Complete monthly reviews of data relevant to the project and provide updates to direct supervisor.
  1. Continually seek the improvement in the quality of the HOC operations and case management program.
  1. Maintain appropriate professional boundaries with clients and co-workers. Ability to adhere to an appropriate self-care plan.
  1. Maintain training to ensure up to date qualifications in best practices such as progressive engagement, motivational interviewing, mental health first aid, harm reduction and others as directed by supervisor.
  1. Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  1. Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.
  1. As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.
  1. Adhere to the tenants of Catholic Social Teaching and Catholic Doctrine.
  1. Perform related functions necessary to support the mission and core values of Catholic Charities.
Job Qualifications

To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.

Education/Experience:to perform this job successfully, an individual must have a bachelor’s degree in social services or related field and minimum one year of relevant experience in direct client services; OR an AA Degree with 3 years of relevant experience in direct client services; professional experiencing working in low-barrier homeless services or the field of mental health services preferred.

Certificates/Licenses:To perform this job successfully, an individual must have a valid Driver’s License and successfully pass background check applicable to position.
