Job description

The Ryan White Non-Medical Case Manager will work closely with clients living with HIV/AIDS who have multiple psychosocial and/or health-related needs. The Non-Medical Case Management services adopt a strength-based approach, including treatment adherence counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining housing, financial support, legal services, social support, and other needed services. The goal is to ensure clients have timely access to comprehensive medical care and social services, prevent disease transmission, delay HIV progression, and promote client independence and self-sufficiency.

Key Performance Areas
  1. Screening: Schedule patients for appointments, support check-out, collaborate with medical providers to identify clients needing case management, complete eligibility screening, respond within 24 hours, and provide feedback to care teams.
  2. Orientation and Assessment: Determine eligibility for services, conduct intake interviews, obtain documentation, coordinate with community agencies, and maintain accurate records and data entry.
  3. Continued Contact: Maintain face-to-face contact every 90 days, document treatment adherence activities, and perform other duties as assigned.
Training and Requirements
  • Knowledge of ADAP, Medicaid, Medicare, SSI, SSDI, HIV standards, legal issues, cultural competency, and eligibility requirements.
  • Participation in staff trainings, educational programs, and continuing education (minimum 5 hours).
Minimum Requirements
  • High School Diploma
  • One year of experience working with persons living with HIV or related experience of two years
  • No licensure required
Additional Details

This position is located in Charlottesville, VA, and is dependent on project needs and funding. It is a non-exempt, benefited position that does not sponsor work visas.
