Community Healthcare Network (CHN) is seeking a Community Health Educator - HIV in New York, NY. This full-time role focuses on educating patients living with HIV/AIDS, reducing transmission, and coordinating with case managers to address social determinants of health in LGBTQ+ communities.

Qualifications

• Two years of experience in community health education or related field.
• Ability to develop, implement and evaluate health education programs.
• Strong interpersonal and written/verbal communication skills.

Responsibilities

• Design, implement and evaluate patient education programs for HIV/RSV populations.
• Conduct health intakes and assessments.
• Provide group and individual sessions on health topics and risk reduction.
• Organize educational materials and maintain inventories.
• Provide psychosocial support and coordinate with case managers and program staff.
• Enter data into charts and medical records timely.
