Job summary

Qualifications

• At least two years experience in a health-related field with patient contact.
• Ability to plan, implement and evaluate health education programs.
• Strong verbal and written communication skills.
• Familiarity with group dynamics and counseling.
• Respect for diverse backgrounds including LGBTQ+ communities.

Responsibilities

• Design, implement and evaluate health education programs for the Ryan White clinic.
• Conduct health intakes and assessments.
• Lead group and individual education sessions on health topics.
• Coordinate with case managers and program staff to address SDOH.
• Manage educational materials and outreach activities.
• Enter data into charts and medical records in a timely manner.
