Job summary

LaSante Health Center is seeking a dedicated HIV Community Liaison to support individuals living with HIV and enhance community engagement. You will connect patients to care, conduct outreach, and educate communities while building partnerships with local organizations.

Ideal candidates have experience in community outreach or healthcare, knowledge of HIV prevention and support services, and strong interpersonal skills. Haitian Creole bilingual ability is preferred.

• Maintain records and complete assigned tasks.
