Parkwestmed in Maryland is seeking a Medical Case Manager to deliver case management and treatment adherence services for the Hidden Garden Program. You confirm Ryan White eligibility, collect required documentation, and coordinate referrals to core medical and support services.

You will monitor care plans, document progress, advocate for clients, and provide crisis intervention when needed. The role requires collaboration with staff, CAB liaison, and monthly education or support group

Qualifikationen

• Master's degree in social work from an accredited program.
• Current Maryland LMSW or LCSW-C licensure required.
• Minimum of two years' social services experience.
• Experience with HIV/AIDS and minority communities is preferred.
• Knowledge of HIPAA and privacy standards.

Aufgaben

• Provide medical case management and treatment adherence support to clients.
• Confirm Ryan White eligibility and ensure documentation.
• Refer clients to primary medical care and coordinate labs and prescriptions.
• Monitor care plans and client progress with signatures.
• Coordinate internal and external referrals to health and support services.
• Provide monthly narratives identifying barriers and solutions.
• Facilitate health education or support groups.
• Liaise with the Consumer Advisory Board (CAB).
