Job summary

Under the direction of the Program Manager, the LVN helps with assessments, care plan implementation, treatment adherence education, referrals, and documentation to ensure program standards and positive health outcomes.

Qualifications

• Graduation from an LVN program and licensure in Texas.
• No experience required.
• Current BLS required.

Responsibilities

• Provide direct nursing care to patients within the scope of vocational nursing practice.
• Assist with client assessments, care plan implementation, and treatment adherence education.
• Coordinate referrals to medical, behavioral health, and supportive services; document accurately.
