BrightStar Care – Cuyahoga West is seeking an LPN with Trach & Vent experience to provide skilled nursing care in home and facility-based settings, including pediatric, memory/dementia, and complex care patients.

The role offers Monday–Friday: 8a-4p with weekend options, RN oversight on all cases, and opportunities for ongoing training and career development. BrightStar Care – Cuyahoga West serves Cleveland's west side with local roots.

Qualifikationen

• Active Ohio RN license (unencumbered).
• Minimum 1 year clinical experience (home care or critical care preferred).
• Strong clinical judgment and independent decision-making skills.
• Current BLS/CPR certification and TB clearance.
• Reliable transportation and valid driver’s license.
• Ability to lift up to 50 lbs and assist with patient mobility.
• Up-to-date vaccinations (COVID-19, Flu, Hep B).

Aufgaben

• Monitor patient condition and respond to clinical changes appropriately.

• Maintain accurate, timely clinical documentation.
• Communicate effectively with care team, patients, and families.
• Ensure compliance with all safety, hygiene, and clinical standards.
• Participate in ongoing training and skills development.

Schedule Options

• Willing to work in Elyria, Lorain
• Monday to Friday: 8a-4pm
• Weekend Support: 8a-4pm
