We are hiring experienced Home Health Physical Therapists for our Home Health Teams operating within South Chicagoland.

Sign-on Bonus: Eligible for up to $10,000, subject to taxes. Must be working in this position at payout. Internal team members are not eligible.

Position Highlights:

Work independently within a reputable organization, assessing and addressing patients' mobility, safety, communication, and equipment needs. Build strong relationships with patients and families, and collaborate with healthcare providers as needed.

Requirements:

• Licensed Physical Therapist in Illinois or eligible for licensure.
• At least 1 year of clinical experience or strong physical therapy affiliation experience.
• Proficient with home therapy equipment and assistive devices.
• Basic computer skills and ability to learn new systems.
• Strong communication and documentation skills.
• Good interpersonal, time management, and self-organization skills.

Responsibilities:

• Travel extensively within the geographical area, in all weather conditions.
• Work rotating weekends and holidays; adhere to scheduled hours.
• Perform physical activities including lifting up to 35 pounds, assisting with patient transfers, and providing treatment in home environments.
• Develop and evaluate personalized therapy programs, promoting patient self-care.

Pay Range: $43.30 - $64.95
