Description

Position Overview

Shelter House is a community of residents, staff and volunteers working together to provide basic human services for the men and women of the Greater Cincinnati area who are experiencing homelessness. The HIP Team Case Manager is responsible assisting the chronically homeless to services and housing.

Essential Job Duties

  • Complete intakes and assessments for each clients in case load
  • Complete Individual Service Plan (ISP)
  • Responsible for connecting clients to a housing program that has availability
  • Refer clients to wrap around services as needed, such as, drug and alcohol treatment, mental health services, etc.
  • Help clients stabilize once they are housed by helping with furniture, food, transportation for services, home visits, etc.
  • Assist clients to get medical benefits
  • Maintain contact with clients for 90 days after they are housed
  • Maintain contact with community service providers to aide with the client referral process
  • Perform housing visits for placed clients
Qualifications

Qualifications & Requirements

  • Bachelor’s degree required
  • LSW/MSW preferred
  • CDCA preferred
  • Experience with case management, homelessness, mental health or substance abuse
  • Since the Case Managers are responsible for driving clients in their personal vehicle, they need to have the following: Valid driver’s license with good driving record, daily access to private insured vehicle during work hours and the ability to be approved by Shelter House insurer.
  • Must be able to work nights, weekends, and/or holidays

Physical Requirements (% of time spent performing requirements)

  • Time spent walking- 15%

Must be able to lift 10lbs

  • Time spent working on computer and telephone- 30%
