Description

Apply for the HIP Case Manager II position at Bloomington Health Center, located at 811 West 2nd Street, Bloomington, IN 47403. The role is classified as exempt under the Fair Labor Standards Act.

The Case Manager II contributes to HIP's mission by coordinating client care based on household strengths, needs, and barriers to achieve sustainable housing and employment or income, with strong community connections. They serve as a point person for systems of care such as addiction, mental health, disability, elderly care, and housing vouchers. Responsibilities include providing client-centered services, supporting team members, training, and linking clients to community resources. Key outcomes involve building rapport, assessing needs, developing and tracking SMART goals, conducting home visits, and maintaining accurate documentation.

Key Expected Outcomes
  • Participate in all mandatory meetings such as staff, team, case management, housing committees, and supervision.
  • Assist clients in collecting eligibility documentation and conduct assessments.
  • Develop comprehensive housing plans with clients and partners, setting SMART goals and monitoring progress.
  • Conduct weekly client meetings and home visits to assess needs and maintain relationships.
  • Connect clients to community resources and participate in supervision and professional development.
  • Train staff on specialty care areas, share resources, and support client housing stabilization plans.
  • Document all client interactions accurately in HMIS and attend outreach activities.
  • Promote positive communication within the team and represent HIP professionally.
Leadership Responsibilities (Optional)
  • Manage project budgets and performance goals.
  • Evaluate service quality and oversee data collection.
  • Lead group sessions and support staff training.

Requirements

Work Environment & Physical Demands
  • Proficiency with computers and office equipment.
  • Work schedule from Monday to Friday, 8 a.m. to 5 p.m., with possible extended hours.
  • Travel within the community required.
  • Reliable transportation and valid driver’s license required.
  • Ability to pass background checks.
