Job summary

The Health Information Management (HIM) Data Abstractor at Northfield Hospital + Clinics reviews, analyzes, and abstracts clinical and demographic information from paper records, CCDs, and EMRs to ensure complete, accurate patient data for patient care, regulatory compliance, and quality initiatives.

Responsibilities include identifying inconsistencies, indexing records in the EMR, maintaining HIPAA privacy, and collaborating with an interdisciplinary team.

Qualifications

• Experience in medical record abstraction, chart review, document imaging, CCD review, or a related healthcare field.
• Must be able to speak, read, and write English proficiently to ensure safe and effective communication in a healthcare environment.

Responsibilities

• Reviews, analyzes, and abstracts pertinent patient information from paper records, external records, CCDs, health information exchanges, and EMRs into the NH+C EMR.
• Ensures abstracted information is accurate, complete, and entered into the correct location within the EMR.
• Identifies, investigates, and resolves inconsistencies, discrepancies, duplicate information, and incomplete documentation.
• Maintains confidentiality and adheres to HIPAA and regulatory requirements.
• Supports data integrity and participates in QA activities.
