Olean General Hospital is seeking a Health Information Clerk I to support the processing, maintenance, and imaging of electronic medical records in compliance with hospital, state, federal, and ISO standards.
The role involves responding to requests for protected health information, ensuring data integrity, scanning documents into the EMR, and coordinating with physicians to resolve incomplete or deficient records while participating in the Health Information Team and ongoing staff development.
It is a position for a Health Information Clerk 11 that supports the processing, maintenance, paper and electronic storage, retrieval, scanning and quality review of electronic medical records in accordance with hospital, state, federal, legal, and ISO standards. It involves responding to and release of protected health information in accordance with regulatory requirements, hospital policies and procedures and federal and state statutes to authorized parties. It maintains integrity of information through the electronic medical record while retrieving confidential patient information. It reviews and evaluates incomplete records and assigns deficiencies (both dictated and electronic signatures). It communicates to physicians and their offices regarding deficient and delinquent records. It prepares records for scanning, verifies patient identification on each page and analyzes for completeness. It scans documents into the electronic document management system and examines pages to ensure image quality and complete quality review indexing (assign form ID). It verifies data integrity. It ensures that paper record is disposed of properly. It prepares birth certificates, acquires appropriate signatures and files information. It prepares, properly executes and files paternity affidavits as necessary. It participates as a member of the Health Information Team along with participation in staff development.
High school graduate or equivalent with related courses in typing, filing, office methods and business required.
6 months of experience working within a hospital or medical office position preferred. Working knowledge of basic computer applications and Microsoft Office suite. Knowledge of Medical Terminology, independent judgment in making decisions which may require intense investigation and interpretation in making decisions.
