Job summary

Garnet Health Medical Center, part of Garnet Health System, is seeking a High Value Care Coordinator to monitor value- and quality-based reimbursement programs in collaboration with practice leaders. This on-site role focuses on improving reimbursements and value of care.

The ideal candidate has 3–5 years in healthcare, proficiency with EMR (Epic preferred) and strong data analysis with Excel and Microsoft Office. A High School GED is required; an associate or bachelor’s degree is preferred.

Qualifications

Responsibilities

POSITION SUMMARY:

The High Value Care Coordinator monitors value/quality based reimbursement programs offered by medical group commercial payers and CMS and works in collaboration with the Practice Administrator and/or Practice Manager at each location to close gaps and increase value based reimbursements.

The compensation range for the role is $62,358 - $77,948.00 annually

Qualifications

POSITION QUALIFICATIONS:

Minimum Education:

High School\GED degree required (Associate\Bachelor preferred)

Minimum Experience:

3-5 years of experience in health care settings, ideally in the areas of direct patient care, case management, care coordination, patient outreach, front desk\clinic reception, and\or quality improvement

Language Skills: Ability to read, write and speak English.

Reasoning Ability: Able to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills: To perform this job successfully, an individual should have knowledge of electronic medical records (experience in Epic preferred) , and Microsoft Office Suite including Excel, MS Word, PowerPoint, Outlook etc. Proficient in data analysis and manipulation (Excel summary tables, charts, pivot tables)
