Job description

Provides coordination of care to patients under the direction of the CHFM Medical Directors. Assists with specific in-basket electronic communication, clinical operations, and counsels patients regarding their health. Virtually assesses patient care and obtains appropriate input as needed to determine by NPP / Physician under the direction of the Clinic Medical Director.

Responsibilities

Essential Functions

1. Organizes and performs all administrative support services for the assigned directors including reserving and scheduling meetings, invoice processing, answering phones, responding to email inquiries, coordinating and ordering food, and preparing meeting areas.
2. Provides government entity checks for vendors, maintains credit applications for vendors, and increases reimbursement to payors, especially Medicare, by ensuring justification for higher acuity coding for encounters, potentially leading to higher reimbursements.
3. Researches patient requests for medical advice or prescription refills, and manages data and orders within pre-approved practice protocols.
4. Schedules and coordinates monthly Finance Committee meetings and Audit Committee meetings, prepares minutes, and compiles monthly finance packages and presentations.
5. Coordinates the yearly Audit Meeting, distributes materials, attends the meeting, and creates minutes for dissemination.
6. Collects and assesses patient data to determine needs and promote optimal care. Communicates with patients via calls and EMR messages regarding care coordination, health maintenance, and appointments after review by clinic provider.
7. Reviews and pends medications, labs, and referrals for patients based on departmental or practice guidelines.
8. Reviews and prioritizes all EPIC in-basket tasks and addresses them appropriately.
9. Assists with follow-up for established patients requiring continuity of care, including timely referrals to specialists and ancillary services, for review and sign-off by the clinic provider.
10. Coordinates education and guidance for patients to maximize outcomes after review by the clinic provider.
11. Performs other tasks as assigned.

2 years outpatient setting preferred.

Education Requirements

High School Diploma or GED required. Bachelor’s in Science or Health-related field preferred.

LPN license required. MA Certification preferred.
