Community Health Care Inc. is seeking a high-risk OB nurse to work in the women's health center, supporting OB providers and managing high-risk pregnancy cases. The role includes patient education, routine vitals, injections, and coordination of referrals in a clinic setting.

The candidate will maintain HR patient records, schedule follow-ups, and ensure clear, concise documentation in the EMR. NJ RN licensure, CPR certification, and OSHA training are required, with emphasis on infection control.

Qualifications

• HS Diploma or equivalent.
• RN licensure in good standing.
• Competent computer skills and EMR familiarity.

Responsibilities

• Review charts of patients scheduled with providers and establish a standardized plan of care.
• Assess high-risk pregnancy factors and provide patient education and support.
• Monitor vital signs and administer injections as indicated (e.g., Rhogam, Hydroxyprogesterone).
• Coordinate referrals and follow-up care, update EMR, and ensure timely documentation.
• Perform routine nurse duties including BP checks and lab-related testing.
• Educate patients on chronic disease management and preventive care.
• Maintain infection control practices and timely communication with patients and providers.
