Qualifications

• Must hold a valid New Jersey CHHA license (Required).
• High school diploma or equivalent.
• Compassionate, reliable, and professional.
• Excellent communication skills; ability to coordinate with families and nurses.
• Ability to work independently in clients' homes.
• Driver’s license and reliable transportation preferred.
• Must pass background check and health screening.

Responsibilities

• Assist clients with daily living activities as prescribed.
• Provide companionship and emotional support.
• Light housekeeping, meal prep, and medication reminders.
• Follow the care plan provided by the nurse.
• Report changes in condition to the agency.
• Maintain accurate documentation.

• Provide companionship and emotional support

• Perform light housekeeping, meal preparation, and medication reminders
