Job summary

Professional Healthcare Resources seeks a compassionate Home Health Aide to join our Hospice team in Baltimore, MD and surrounding areas. This PRN position supports patients at home under RN supervision and follows a Plan of Care.

Responsibilities include providing quality personal care, adhering to agency policies, and complying with federal, state, and local guidelines. English proficiency and reliable, compassionate care are essential.

Qualifications

• High school diploma or equivalent required or preferred.
• Successful completion of state-approved CNA program meeting Medicare/Medicaid standards.
• Minimum One Year experience as a Home Health Aide for Hospice patients.

Job Description

A care-giving professional providing quality home-based personal care and other related services in accordance with established agency policies and procedures; functions under the supervision, direction, and instruction of a Registered Nurse, and in accordance with an approved Plan of Care; complies with federal, state and local guidelines for all services.

You Must Possess The Following Qualifications And Experience

• High school diploma or equivalency preferred, but not required
• Successful completion of state-approved CNA program that meets time and content requirements consistent with Medicare/Medicaid standards
• Minimum One Year experience as a Home Health Aide for Hospice patients
• Meets standards of evaluation of competency and continuing education as defined in federal statutes
• Good command of the English Language
• Good problem solving skills
