Position: Home Health Aide (HHA)

Salary: $12 - $17 / per hour

Position Description:

The Home Health Aide (HHA) is a member of the home care team who works under the supervision of a Registered Nurse or Therapist. The HHA performs various personal care services, as necessary, to meet the patient’s needs. The HHA provides service in accordance with Maxim’s philosophy, standards, and policies.

Essential Duties and Responsibilities:

• Observes and documents patient condition and reports any changes to the assigned nurse supervisor.
• Communicates clearly and effectively with the client, family/caregiver, and office.
• Successfully completes Maxim’s Competency Program.
• Practices within scope of certificate/license.
• Follows the patient’s plan of care and completes all assigned tasks.
• Adheres to Maxim’s policies, procedures, documentation standards, and professional boundaries.
• Maintains a commitment to quality and competent patient care.
• Completes all assigned trainings in a timely manner.
• Performs any other duties as assigned.

Minimum Requirements:

• Meets all training requirements according to state and federal laws.
• Ability to read, follow written instructions, and document care.
• Current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.
• Meets all federal, state, and local requirements.
• Proficiency in English.
• Excellent communication skills.
• Technological proficiency, including mobile documentation devices.
