POSITION SUMMARY:
As the HHA Coordinator you will assist the Case Manager with an assigned group of patients and
coordinate Home Health Aide (HHA) level care between Case Manager, HHA, and Client. The HHA
Coordinator assists with scheduling, in-services, and implementing plans of care. Additionally, they
assist with interviewing and hiring Home Health Aides. This role assists the Area Manager/Director with
various tasks, as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Prepare and assemble HHA packets
2. Assist in hiring and delivering orientation to HHAs
3. Assist with in-service, incidents, and terminations of all HHAs
4. Train HHAs on IPOC charting
5. Review and ensure all HHA paperwork is complete after orientation
6. Review IPOC and Paper notes for accuracy and compliance
7. Scan, label and upload all HHA contractor paperwork to corresponding platform
8. Schedule visits in IGEA for HHA charting
9. Prepare, order and deliver new Client care kits and binders
10. Maintain updated Client/HHA list with HHA schedules
11. Resolve, schedule and communicate replacements for HHA call-ins, fulfilling HHA shifts as needed
12. Ensure 24-hour patient schedules are updated
13. Maintain tracking system for HHA notes ensuring timeliness and compliance
14. Schedule HHA hours
15. Collaborate with Case Managers regarding Clients’ HHA needs
16. Notify all HHAs about updated procedures
17. Coordinate HHA Plan of Care with Case Manager
18. Follow all Agency policies and procedures
19. Perform other duties as assigned
OTHER DUTIES AND RESPONSIBILITIES:
This position may require traveling between locations in your personal vehicle
COMPENSATION
We offer competitive pay at a range of $18.00-20.00
SUPERVISORY RESPONSIBILITY:
This position has no supervisory responsibilities
PREFERRED QUALIFICATIONS:
1. High school diploma or equivalent
2. CNA training may be required, or preferred
3. Excellent communication and interpersonal skills
4. Strong organizational and time-management skills
5. Proficiency in Microsoft Office Suite
6. Must pass a criminal background check and drug screen
7. Professional appearance required
WORK ENVIRONMENT and PHYSICAL REQUIREMENTS:
1. Ability to sit at a computer terminal for an extended period of time.
2. While performing the duties of this job, the employee is regularly required to, stand, sit; talk, hear,
and use hands and fingers to operate a computer and telephone, keyboard, reach, stoop and kneel.
3. Specific vision abilities required by this job include close vision requirements due to computer
work.
4. Minimal lifting of office records and paperwork.
5. Regular, predictable attendance is required as business demands dictate.
