Job Summary:

We are seeking a compassionate Home Health Aide/Caregiver/CNA to join our team. In this role, you will work directly with patients, following personalized care plans in their homes. The ideal candidate operates with a big heart, providing attentive care while building genuine relationships. You should be detail-oriented, organized, and committed to creating a healthy environment that meets patient needs. Strong communication skills are essential. Prior experience in home health or hospice care is a plus.

Responsibilities:

  1. Assist patients with daily activities such as moving, bathing, and transportation.
  2. Perform personal care, change bed linens, do laundry, and clean the home.
  3. Maintain records of patient care, condition, and progress, and communicate observations with supervisors.
  4. Build relationships with patients through conversation or reading aloud to promote mental health and alertness.
  5. Provide medication reminders.
  6. Prepare light meals.
  7. Engage patients in exercises or activities.

Qualifications:

  • Graduated from an accredited Home Health Aide program.
  • High School Diploma or GED.
  • At least one year of professional experience.
  • Valid driver’s license.
  • CPR certification.
