Qualificações

• Proven working experience as home health aide with proof of HHA and CNA certificate.
• Ability to monitor vital signs and document observations.
• Familiarity with End-of-Life care and basic nutrition/hygiene standards.

Responsabilidades

• Provide health care services in patients residences (homes, assisted living facilities, board & care homes and SNFs).
• Perform light domestic and household tasks as needed.
• Assist with client’s personal care activities and mobility improvements.
• Monitor patients’ vital signs and report on their condition.
• Maintain patient care records and document services provided.
• Instruct and counsel patients and families on diet and exercise.
• Provide companionship and emotional support.

Due to growth, we are looking for an experienced Certified Hospice Aid for Stanislaus and Merced Counties to join our dynamic team.

Responsibilities

• Provide health care services in patients residences (Homes, Assisted Living Facilities, Board & Care Homes and Skilled Nursing Facilities)
• Perform light domestic and household tasks
• Assist with client’s personal care activities
• Monitor patients (vital signs, temperature, respiration, etc.) and report on their condition
• Maintain patients care records and document provided services
• Assist patients with mobility and exercise as prescribed
• Instruct and counsel patients and families on diet and exercise
• Provide companionship and basic emotional or psychological support

Skills

• Proven working experience as home health aide with proof of HHA and CNA certificate
• Ability to monitor vital signs
• Familiarity with basic nutrition and personal hygiene standards
• Familiarity with End-of-Life care, including post-mortem care
• Proficiency in English, ability to read and follow written instructions and document care given
• Caring and compassionate personality
• Current CPR certificate
• First Aid training
• Valid driver’s license and/or reliable transportation
• High school diploma or equivalent
• Bilingual Spanish-speaking strongly preferred
