Job summary

BHcare seeks a HERO Case Manager to join the Housing Program (HERO) in Connecticut. This full-time role focuses on outreach, engagement, and supportive housing pathways to promote stability and well-being.

You will build relationships with clients, connect them to housing, employment, healthcare, and community services, and collaborate with landlords and BHcare teams. Travel throughout the Greater New Haven area is required; CT residency is necessary.

Qualifications

  • High school diploma or equivalent required; bachelor’s degree preferred.
  • Experience working with individuals experiencing homelessness, housing instability, substance use disorders, and/or mental health challenges.
  • Understanding of Housing First principles and commitment to person-centered, strengths-based services.
  • Knowledge of housing, employment, and human service resources within the Greater New Haven community.

  • Help clients identify, obtain, and maintain safe and affordable housing opportunities.
  • Assist individuals in developing personalized goals that promote housing stability, wellness, and independence.

  • Collaborate with landlords, housing providers, community agencies, and internal BHcare teams to support positive client outcomes.
  • Provide transportation assistance and support clients during transitions or times of increased need.

Job Location: Shoreline Adult Services North Branford, CT - North Branford, CT 06471, Position Type: Full Time, Education Level: High School, Salary Range: $23.00 - $24.50 Hourly, Travel Percentage: Up to 25%, Job Shift: Day, At BHcare, a nonprofit organization, we believe everyone deserves the opportunity to live a healthy, safe, and fulfilling life. Every day, our teams provide compassionate care and support to individuals and families across Connecticut through behavioral health, substance use, domestic violence, housing, and community-based services.

We are looking for a HERO Case Manager who is passionate about making a difference and wants to join a team committed to helping people thrive.

This is a full time, 37.5 hours per week position in our Housing Program (HERO). Flexible schedule based on client and program needs, including community-based outreach and client support. Travel throughout the Greater New Haven area is required.

Candidates must reside in Connecticut to be eligible for this position.

What You Will Bring
Required Qualifications
  • High school diploma or equivalent required; bachelor's degree preferred.
  • Experience working with individuals experiencing homelessness, housing instability, substance use disorders, and/or mental health challenges.
  • Understanding of Housing First principles and commitment to person-centered, strengths-based services.
  • Knowledge of housing, employment, and human service resources within the Greater New Haven community.
  • Proficiency in Microsoft 365 and the ability to navigate electronic health record (EHR) systems.
  • Strong communication, relationship-building, and problem-solving skills with the ability to work effectively with diverse populations.
  • Valid driver's license, acceptable driving record, and ability to transport clients as needed.
Preferred Qualifications
  • Bilingual English/Spanish skills.
  • Training in harm reduction, motivational interviewing, and de-escalation techniques.
  • Experience working in housing, homeless outreach, behavioral health, substance use treatment, or community-based human services programs.
  • Experience coordinating services and navigating community resources for vulnerable populations.
What We Offer

The anticipated salary range for this position is $23.00 - $24.50. Actual compensation offered will be determined based on a variety of factors including relevant skills, experience, qualifications, certifications, and internal equity considerations.
