Qualifications

• Two years experience in community engagement, community organizing, and project coordination and implementation.

Responsibilities

• Investigates reported Hepatitis C cases and provides education, testing, and referral services to enhance patient engagement.
• Maintains detailed, accurate, and confidential records in health surveillance systems.
• Provides health information to the public and makes referrals to appropriate agencies.
