Job summary

Hartford HealthCare is seeking an Oncology Nurse Practitioner or Physician Assistant to join our growing team in Hartford, CT, providing value-based outpatient oncology care.

The role emphasizes triage, urgent visits, pre-treatment assessments, and survivorship care, with opportunities for professional advancement and mentoring within the Cancer Institute. A CT license as PA or APRN, and oncology experience, are required, with 1 year of APP experience preferred.

Qualifications

• Licensed CT PA or APRN with current credentials.
• Oncology experience as PA/NP/RN is required.
• CTPA/APRN certification is required for practice in CT.
• Minimum 1 year of Advanced Practice Provider experience preferred.

Responsibilities

• Triage and manage outpatient oncology patients, including urgent visits and pre-treatment assessments.
• Provide follow-up and survivorship care for oncology patients.
• Collaborate with oncologists and the care team to coordinate value-based care.
• Engage in patient education, care planning, and symptom management.

Qualifications

• Eligibility to be licensed in Connecticut as a Physician Assistant (PA), or Advanced Practice Registered Nurse (APRN), is required.
• Eligibility to be certified as a Physician Assistant (PA), or Nurse Practitioner is required.
• Oncology experience as a PA, NP, or RN is required.
• A minimum of 1 year of experience practicing as an Advanced Practice Provider (APP) is highly preferred.
