Job summary

Responsibilities include interdisciplinary care planning, family involvement, and ongoing documentation. 6 months HD experience preferred; ADN required with BSN preferred. Cincinnati, OH location, full-time, day shift.

Qualifications

• Current OH RN License and eligibility to practice nursing in Ohio.
• Minimum 6 months of Hemodialysis experience.
• Graduate from an accredited school of nursing (ADN required; BSN preferred).

Responsibilities

• Prepares, implements, evaluates, revises, and coordinates interdisciplinary plan of care for each patient.
• Communicates plan of care to all levels of staff and prioritizes care based on patient needs.
• Provides patient and family with clear treatment instructions and includes them in ongoing care.
• Addresses behavioral health, cultural, or religious considerations impacting care.
• Demonstrates critical thinking, accountability, flexibility, and teamwork in patient care.
• Documents nursing process accurately per policy and procedures.
• Acts as a resource and mentor for new staff and floating team members.
• Contributes to ongoing professional development of peers and self.
