Qualifications

• Active IN License and DEA or IMLC LOQ.

Responsibilities

• Coverage needed Monday-Friday, 8:00am-4:30pm.
• Outpatient setting with patient volume of 20-25 patients per day.

Job description

• Coverage Needed:
  • As soon as credentialed - ongoing
  • Monday-Friday, 8:00am-4:30pm
  • Clinic + call
• Clinical Information:
  • Outpatient setting
  • EMR: EPIC
  • No Call or rounding
  • Skills/Procedures: Knowledge of NCCN guidelines, Emergency response skills from side effects of medication, Chemotherapy treatment plans
  • Patient Volume: 20-25 patients per day
  • Support Staff: 5 RN's, 2 MA's, 1 Patient Service Representative
• Required:
  • CV must be in Month / Year format
  • Must have license in hand at time of submission, or proof that the license application has been submitted
  • Any gaps greater than 30 days will require an explanation on the CV
  • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
  • Active IN License and DEA or IMLC LOQ
  • Board Certified in Hematology/Oncology
