Job Overview

As a Hematology/Oncology Clinical Pharmacist, you will oversee the provision of safe and effective medication therapies through collaboration with the patient’s care team. Your responsibilities include reviewing physician orders for drug therapy, monitoring therapy for drug choice, dosage, drug interactions, adverse reactions, and formulary adherence. You will also serve as a drug information resource to nurses, physicians, and other healthcare workers.

Essential Functions
  1. Retrieve and evaluate relevant biomedical literature, formulate responses, and provide concise, timely drug information.
  2. Use effective patient education techniques to counsel patients and caregivers on medication therapy, adverse effects, compliance, and administration.
  3. Communicate unresolved issues during shifts for future action.
  4. Gather information to design medication therapy regimens and identify medication-related problems.
  5. Design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens, implementing recommendations effectively and documenting in medical records.
  6. Participate in medical emergency management.
  7. Precept pharmacy students and residents.
  8. Prepare and dispense medications following best practices, policies, and regulations, managing therapies using clinical guidelines.
  9. Check accuracy of work by pharmacy support team members according to policies and procedures.
Physical Requirements

Work primarily in the pharmacy, deliver medications to nursing units, and perform physical activities such as walking, standing, pushing, lifting, stretching, bending, and kneeling. Use PPE to minimize exposure to hazardous agents, and take precautions against needle sticks and cuts. Must be able to perform manipulative skills, see clearly, communicate effectively, and hear normal sounds. Rotation shifts are involved.

Education, Experience, and Certifications

Bachelor's Degree in Pharmacy or PharmD from an accredited school is required. A pharmacist license issued by the practicing state is required. Previous hospital pharmacy experience is preferred. Must have effective verbal and interpersonal communication skills. Immunization certification is required.
