As a Hematology/Oncology Clinical Pharmacist, you will oversee the provision of safe and effective medication therapies through collaboration with the patient’s care team. Your responsibilities include reviewing physician orders for drug therapy, monitoring therapy for drug choice, dosage, drug interactions, adverse reactions, and formulary adherence. You will also serve as a drug information resource to nurses, physicians, and other healthcare workers.
Work primarily in the pharmacy, deliver medications to nursing units, and perform physical activities such as walking, standing, pushing, lifting, stretching, bending, and kneeling. Use PPE to minimize exposure to hazardous agents, and take precautions against needle sticks and cuts. Must be able to perform manipulative skills, see clearly, communicate effectively, and hear normal sounds. Rotation shifts are involved.
Bachelor's Degree in Pharmacy or PharmD from an accredited school is required. A pharmacist license issued by the practicing state is required. Previous hospital pharmacy experience is preferred. Must have effective verbal and interpersonal communication skills. Immunization certification is required.
