Job Title: Patient Care Advocate

Duration: 6 months Contract (Potential to Extend) FTE

Location: Remote – OH (Reside in OH – Cleveland, Columbus, Toledo, Findlay, Athens, Cambridge, Lima, Bowling Green or nearby)

Shift: 9:00 am – 5:00 pm EST (with flexibility for later outreach hours)

Responsibilities:

• Outreach to address HEDIS gaps, reduce barriers to care (e.g., scheduling, transportation, education on benefits and preventive care)
• Communicate daily with members to close HEDIS care gaps and meet/exceed scheduling goals
• Work independently in a remote setting, maintaining friendliness and motivation
• Visit provider offices within an hour of home for scheduling assistance as needed

First Day Overview:

• Post-onboarding, meet with hiring manager for orientation on project goals and reporting structure
• Systems training on software, tracking, and specifications

Previous Experience/Background:

• HEDIS outreach, abstractor, RN/LPN/SW, customer service roles
• Bachelor’s degree preferred

Required Skills:

• Proficiency in Microsoft Excel
• Comfortable communicating with members and providers
• 2+ years in healthcare or quality roles
