Qualifications

• Bachelor's degree in nursing required or in progress.

• Strong communication, leadership, and analytical abilities.

Responsibilities

• Lead programmatic development for Heart and Vascular programs through care coordination and outreach management.
• Serve as the primary clinical-expertise contact and participate in care-pathway development.
• Cultivate, implement, and monitor evidence-based standards of care.
• Coordinate interdisciplinary care and post-discharge education for patients and families.
• Assess patients, collect and analyze clinical data, document nursing interventions and care plans.
• Facilitate communication among referring hospitals, providers, and service lines.
• Provide community education and assigned training.
• Ensure readiness and compliance with regulatory and Joint Commission requirements.
• Implement strategic initiatives and support cost-of-care improvements.
• Participate in program advancement forums and stay updated on best practices.
