Responsibilities

• Serve as primary clinical expert for the assigned program.
• Collaborate with interdisciplinary teams to advance patient outcomes.
• Ensure post-discharge education and follow-up with patients and families.
• Maintain knowledge of regulatory standards and Joint Commission guidelines.
