Mayo Clinic Arizona seeks a transplant social worker to deliver psychosocial care across advanced heart failure, LVAD, and transplant pathways. You will work with a multidisciplinary team to support patients and families throughout evaluation, waitlisting, hospitalization, and long-term follow-up.

Requirements include an MSW from an accredited program, 2 years post-MSW hospital experience, and an active LMSW license in AZ. Transplant experience is preferred and ongoing education is encouraged.

Qualifications

• MSW degree from an accredited Master of Social Work Program and 2 years of post-MSW social worker experience in a hospital setting are required.
• Active LMSW license through the Arizona State Board of Behavioral Health Examiners is required.
• Transplant social work experience preferred, or willingness to pursue.

Responsabilités

• Provide psychosocial care across heart failure, LVAD, and heart transplant settings.
• Support patients and caregivers from initial evaluation through candidacy review, waitlist management, transplant hospitalization, and lifelong follow-up.
• Navigate insurance, disability, and financial assistance resources with clarity and empathy.
• Collaborate as a core member of a multidisciplinary transplant team.
