Job summary

Mayo Clinic Arizona seeks a transplant social worker to deliver psychosocial care to patients across heart failure, LVAD, and transplant pathways. You will work within a collaborative team, providing support from evaluation through candidacy review, waitlist management, transplant hospitalization, and long-term follow-up.

Requirements include an MSW from an accredited program, 2 years post-MSW hospital experience, and an active LMSW license in Arizona.

Qualifications

• MSW degree from an accredited program.
• 2 years post-MSW social worker experience in a hospital setting.
• Active LMSW license via Arizona State Board of Behavioral Health Examiners.
• Transplant social work experience preferred, or willingness to pursue certification.

Responsibilities

• Deliver psychosocial care to patients across heart failure, LVAD, and transplant pathways.
• Support patients and caregivers from evaluation through candidacy review and waitlist management.
• Assist with transplant hospitalization and lifelong post-transplant follow-up with empathy and clarity.
• Coordinate with multidisciplinary team to address insurance, disability, and financial assistance needs.
