You will support patients and families from evaluation through candidacy review, waitlist management, transplant hospitalization, and lifelong post‑transplant follow‑up, addressing insurance and financial resources with empathy.

Qualifications

• MSW degree from an accredited Master of Social Work Program.
• 2+ years post-MSW social worker experience in a hospital setting.
• Active LMSW license through the Arizona State Board of Behavioral Health Examiners.
• Transplant social work experience preferred.
• Certification in Transplant Social Work (CCTSW) preferred, or willingness to pursue.

Responsibilities

• Provide psychosocial care across heart failure, mechanical support, and transplant care.
• Support patients and caregivers from initial evaluation through candidacy review and waitlist management.
• Navigate insurance, disability, and financial assistance resources with clarity and empathy.
• Collaborate with a multidisciplinary team in inpatient and outpatient settings.
